Glaucoma treatment suitability criteria

Glaucoma is a group of diseases that damage the optic nerve of the eye, causing a gradual loss of vision. It typically affects peripheral vision, but this loss of vision mainly occurs late in the development of the disease, which makes early diagnosis and treatment vital.
There is no specific diagnosis test for glaucoma, so several investigative tests by our professional eye specialists are necessary to identify your condition. An initial clinical examination of specific parts of your eye is usually carried out. Several glaucoma tests exist, such as visual field testing, which involves a blinking light test to identify and measure the disease. Pachymetry is performed to test the thickness of the central cornea to help adjust for intraocular pressure measurements. Optic Nerve Scanning (OCT) is a highly sensitive optical scan that can potentially identify the disease before they are clinically detectable by a doctor.

As with tests, several treatment options for glaucoma also exist. Eye drops are a common, non-invasive treatment that safely reduces pressure in the eye. Selective Laser Trabeculoplasty (SLT) is another method of lowering intraocular pressure in the eye, typically suggested for patients who have had little success with drops. It involves small pulses of lasers being selectively fired at specific cells. A third option is to have a deep sclerectomy. This minimally invasive surgery requires a highly meticulous surgical technique, but it is highly effective and rates of complications are low.

The standard procedure for glaucoma treatment is trabeculectomy. This involves creating full-thickness cuts into the eye in order to drain fluids. Though the risk profile is higher than that of deep sclerectomy, it is effective for the majority of patients. Tube implants may be suggested to reduce eye pressure in complex cases where previous surgery has failed. A range of tube implants are available depending on the patient’s condition.

Glaucoma treatment risks vary by method. Medication can cause systemic side effects; laser therapy might lead to temporary eye pressure increases or inflammation; surgery may result in infection, bleeding, or vision changes. While rare, these risks underscore the importance of specialist care and close monitoring post-treatment.

Eye drops
A number of medications can be prescribed to help treat Glaucoma. These eye drops are intended to reduce the elevated intraocular pressure and prevent damage to the optic nerve. To achieve this, the eye drops can either help the fluid in your eyes drain more effectively or reduce the amount of fluid produced.

Selective Laser Trabeculoplasty
Selective Laser Trabeculoplasty (Commonly referred to as SLT) uses a laser that works at very low levels. It treats specific cells “selectively” leaving the untreated portions of the trabecular meshwork intact. For this reason, SLT may be safely repeated. SLT can be an alternative Glaucoma treatment option for those who have had little success with pressure-lowering eye drops.

Deep sclerectomy
Deep Sclerectomy has a safety advantage compared with standard filtration surgery. This is primarily because the drainage area is created without entering into the eye. A meticulous surgical technique is required. However, its efficacy and low rates of complications, particularly in surgically-induced cataracts, are very appealing to patients.

Trabeculectomy
In Trabeculectomy, a full-thickness cuts into the eye allowing the fluid inside to drain through a flap created from the white of the eye (Sclera). Trabeculectomy is considered the “standard” operation for glaucoma cases. It works very well for the majority of patients, but the risk profile is higher than that of Deep Sclerectomy.

Glaucoma treatment aims to halt further vision loss by managing eye pressure. Although it can’t restore already lost vision, effective management can significantly slow the disease’s progression. Patients often experience stabilised vision, with a marked reduction in the risk of further damage to the optic nerve. Treatment outcomes vary, depending on the glaucoma type, stage at diagnosis, and adherence to prescribed treatments. Regular monitoring and adjustments in treatment plans are crucial for maintaining optimal eye health. Success hinges on early detection, consistent treatment, and patient engagement in managing this chronic condition, helping to preserve quality of life.
