What Is Blepharitis?

Blepharitis is a very common condition affecting the eyelids and eyelashes. Although rarely sight-threatening, it can be uncomfortable, cosmetically bothersome, and closely linked with dry eye disease.

The eyelid margins house the meibomian glands, which secrete oil into the tear film. In blepharitis, these glands, along with the surrounding skin and lash roots , become inflamed or clogged. Bacteria and debris accumulate at the base of the lashes, forming tiny scales or crusts.

Blepharitis is typically a long-term condition, with periods of relative quiet and phases of flare-up. It often coexists with skin conditions such as rosacea or seborrhoeic dermatitis, and may contribute to recurrent styes or chalazia.

It is not caused by poor hygiene in the conventional sense, but consistent lid care is an important part of successful long-term management. Understanding the condition and establishing a daily routine can significantly reduce the frequency and severity of flare-ups.

Recognising the Signs

Blepharitis symptoms are often worse in the morning and may fluctuate with flare-ups. Here is what to look out for.

Red, irritated, or swollen eyelids especially along the lash line
Gritty, burning, or sore eyes
Eyelids sticking together on waking, requiring gentle cleaning before opening comfortably
Crusting or scaling at the base of the eyelashes
Fluctuating vision due to associated dry eye
Watery eyes, particularly in wind or cold air
Recurrent small lumps or cysts at the lid margin (styes or chalazia)
Sensitivity to light and general eye discomfort

What Causes Blepharitis?

Blepharitis arises from a combination of factors. Meibomian gland dysfunction plays a central role, but several other contributors are recognised.

Blockage or dysfunction of the oil-secreting glands in the eyelid margins disrupts the tear film and causes inflammation

Bacterial overgrowth

Excess bacteria around the lash roots contribute to crusting, irritation, and inflammation

Rosacea

A strong association exists between blepharitis and this chronic skin condition

Seborrhoeic dermatitis

Oily, flaky skin affecting the scalp and face can contribute to eyelid margin inflammation

Cosmetic and skincare products

Sensitivities to products used around the eyes can aggravate the condition

Dry eye disease

Blepharitis and dry eye frequently coexist and can worsen each other

Diagnosis and Assessment

Diagnosis is made by examining the eyelid margins, lashes, and meibomian gland openings at the slit lamp. The tear film is also evaluated, as blepharitis and dry eye frequently occur together and both may require treatment.

The eyelid margins, lashes, and meibomian gland openings are examined under magnification to assess the pattern of redness, crusting, and gland blockage.

Meibomian gland assessment

The quality and expressibility of meibomian gland secretions are assessed to determine the degree of gland dysfunction contributing to the condition.

Tear film evaluation

The stability and quality of the tear film are assessed, as blepharitis-related meibomian gland dysfunction is a common cause of evaporative dry eye.

Assessment of associated skin changes

Any associated skin conditions such as rosacea or seborrhoeic dermatitis are noted, as treating these may improve eyelid inflammation.

When to Seek Advice

You should seek specialist review if persistent eyelid irritation, redness, or crusting has not responded to basic lid hygiene measures, if you have frequent styes or chalazia, or if blepharitis is significantly affecting your comfort or vision.

Assessment is also important before certain eye surgeries, including cataract surgery and laser eye surgery, as uncontrolled blepharitis can affect healing and surgical outcomes.
