Read About Our Happy Patients

I saw Professor Jeeva today for a consultation regarding lower blephoroplasty. I was made very welcome and put at ease. The procedure was explained to me and I was encouraged to ask as many questions I wanted. I wasn't made to feel rushed or pressured into anything and left feeling that if I do decide to proceed I will be in safe hands.

My mum Maureen Astles had a Zoom consultation with Mr Jeeva this morning. What a lovely surgeon he was helpful informative and such a nice smile putting her totally at ease. She is now looking forward to meeting him in person for surgery as she knows she will be in the best hands for a successful outcome.

Catherine Astles

I would like to extend my heartfelt thanks to Mr Jeeva, my surgeon and all the doctors and staff at the centre. The care I received from start to finish was exceptional. I felt at ease and was reassured throughout the procedure, feeling safe the entire time. I cannot thank you enough and will definitely be recommending your services to family and friends.

What Are Eye Bags & Wrinkles?

Eye bags and eyelid wrinkles refer to the puffiness, hollowing, sagging skin, and fine lines that develop around the eyes as part of natural ageing. These changes occur when fat pockets shift forward, the skin loses elasticity, and the supporting eyelid structures weaken over time.

While primarily cosmetic, eye bags and wrinkles can cause a tired or aged appearance, dark shadowing beneath the eyes, and heaviness around the lower lids that affects confidence and self-image. Symptoms may worsen with fatigue, stress, allergies, fluid retention, and advancing age.

Some individuals develop significant puffiness, asymmetry, or creasing from an early age due to hereditary fat prolapse. Specialist assessment determines whether conservative management or surgical correction offers the best outcome.

Geneticfactors are the strongest risk driver, some individuals develop pronounced eye bags from an early age
Surgicalcorrection through blepharoplasty offers lasting improvement when non-surgical options are no longer effective

Symptoms of Eye Bags & Wrinkles

The appearance of eye bags and wrinkles varies widely between individuals. Symptoms may be static or fluctuate with lifestyle factors such as sleep, hydration, and allergies.

Puffy or swollen lower eyelids
Visible under-eye fat pads or bulging
Fine lines and creases around the eyes
Loose or sagging lower eyelid skin
Dark shadows beneath the eyes
A tired or aged facial appearance despite adequate sleep
Hollowing or a tear-trough deformity beneath the lower lid
Asymmetry in the appearance of both eyes

Causes & Risk Factors

Eye bags and wrinkles arise from a combination of structural, genetic, and lifestyle factors. Understanding the primary cause guides the most effective treatment approach.

Age-Related Fat Prolapse

As the orbital septum and supporting periorbital tissues weaken with age, the fat pads that normally cushion the eye shift forward and become visible as puffiness beneath the lower lids.

Skin Ageing and Loss of Elasticity

Collagen and elastin depletion in the eyelid skin leads to thinning, wrinkling, and loss of the firmness that keeps skin smooth and supported. This process accelerates from the fourth decade onwards.

Muscle Laxity (Orbicularis Oculi)

Weakening of the orbicularis oculi, the muscle encircling the eye, reduces the tone and support of the lower eyelid, allowing fat to prolapse and skin folds to develop over time.

Genetics

Family history is the strongest single risk factor for eye bags. Some individuals inherit a predisposition to early fat prolapse or thin, lax periorbital skin, resulting in prominent eye bags from a young age.

Lifestyle Factors

Insufficient sleep, high dietary salt intake, dehydration, alcohol consumption, and smoking all accelerate periorbital ageing and worsen transient or structural puffiness around the eyes.

Allergies and Chronic Eye Rubbing

Allergic eye disease causes periorbital swelling and triggers habitual eye rubbing, which progressively stretches and damages the delicate eyelid skin and supporting structures over time.

Sun Exposure

Ultraviolet radiation is a significant accelerator of collagen breakdown and photoageing in the periorbital region. Cumulative UV exposure worsens wrinkling, skin laxity, and pigmentation around the eyes.

Diagnosis and Assessment

A thorough periorbital assessment determines whether the primary issue is fat-based, skin-based, or structural, and defines whether non-surgical or surgical treatment will produce the best outcome for each individual patient.

01

Skin Laxity and Fat Assessment

The extent of fat prolapse, skin redundancy, hollowing, and tear-trough deformity is evaluated clinically. This determines whether the primary issue is fat-based, skin-based, or a combination of both.

02

Eyelid Tone and Position

Orbicularis muscle tone, lower lid laxity, and eyelid position are assessed to establish the structural contribution to the cosmetic change and to plan the most appropriate corrective approach.

03

Review of Contributing Factors

Allergies, fluid retention, systemic causes of puffiness, and lifestyle factors are reviewed and addressed where possible. Non-structural contributors are identified before surgical options are considered.

04

Cosmetic Goals and Treatment Planning

Standardised clinical photography is taken and the patient's cosmetic goals are discussed in detail. The assessment defines whether treatment should be non-surgical (skincare, fillers) or surgical (blepharoplasty).

When to Seek Advice

You may benefit from expert review if you notice any of the following:

  • Persistent puffiness that is not related to fluid retention or fatigue
  • Worsening wrinkles or skin laxity around the eyes
  • Difficulty applying makeup evenly due to creasing or sagging
  • Consistently appearing tired despite adequate sleep
  • Asymmetric eyelid ageing that affects your appearance or confidence
  • Non-surgical methods, skincare, fillers, lifestyle changes, are no longer providing adequate improvement
“Cosmetic concerns about the eye area are entirely valid. A specialist assessment establishes whether non-surgical or surgical treatment best addresses your individual anatomy, concerns, and goals.”
