Eye bags and eyelid wrinkles refer to the puffiness, hollowing, sagging skin, and fine lines that develop around the eyes as part of natural ageing. These changes occur when fat pockets shift forward, the skin loses elasticity, and the supporting eyelid structures weaken over time.
While primarily cosmetic, eye bags and wrinkles can cause a tired or aged appearance, dark shadowing beneath the eyes, and heaviness around the lower lids that affects confidence and self-image. Symptoms may worsen with fatigue, stress, allergies, fluid retention, and advancing age.
Some individuals develop significant puffiness, asymmetry, or creasing from an early age due to hereditary fat prolapse. Specialist assessment determines whether conservative management or surgical correction offers the best outcome.

Causes & Risk Factors
Eye bags and wrinkles arise from a combination of structural, genetic, and lifestyle factors. Understanding the primary cause guides the most effective treatment approach.
Age-Related Fat Prolapse
As the orbital septum and supporting periorbital tissues weaken with age, the fat pads that normally cushion the eye shift forward and become visible as puffiness beneath the lower lids.
Skin Ageing and Loss of Elasticity
Collagen and elastin depletion in the eyelid skin leads to thinning, wrinkling, and loss of the firmness that keeps skin smooth and supported. This process accelerates from the fourth decade onwards.
Muscle Laxity (Orbicularis Oculi)
Weakening of the orbicularis oculi, the muscle encircling the eye, reduces the tone and support of the lower eyelid, allowing fat to prolapse and skin folds to develop over time.
Genetics
Family history is the strongest single risk factor for eye bags. Some individuals inherit a predisposition to early fat prolapse or thin, lax periorbital skin, resulting in prominent eye bags from a young age.
Lifestyle Factors
Insufficient sleep, high dietary salt intake, dehydration, alcohol consumption, and smoking all accelerate periorbital ageing and worsen transient or structural puffiness around the eyes.
Allergies and Chronic Eye Rubbing
Allergic eye disease causes periorbital swelling and triggers habitual eye rubbing, which progressively stretches and damages the delicate eyelid skin and supporting structures over time.
Sun Exposure
Ultraviolet radiation is a significant accelerator of collagen breakdown and photoageing in the periorbital region. Cumulative UV exposure worsens wrinkling, skin laxity, and pigmentation around the eyes.
Diagnosis and Assessment
A thorough periorbital assessment determines whether the primary issue is fat-based, skin-based, or structural, and defines whether non-surgical or surgical treatment will produce the best outcome for each individual patient.
Skin Laxity and Fat Assessment
The extent of fat prolapse, skin redundancy, hollowing, and tear-trough deformity is evaluated clinically. This determines whether the primary issue is fat-based, skin-based, or a combination of both.
Eyelid Tone and Position
Orbicularis muscle tone, lower lid laxity, and eyelid position are assessed to establish the structural contribution to the cosmetic change and to plan the most appropriate corrective approach.
