Glaucoma drops are prescription eye drops that work by either reducing fluid production inside the eye or improving its drainage, helping to lower intraocular pressure and prevent further damage to the optic nerve. With the right combination and consistent use, glaucoma drops can effectively slow down or stabilise the condition over time.
How Glaucoma Drops Work
Glaucoma eye drops are designed to lower the pressure inside the eye (intraocular pressure), which is the main cause of optic nerve damage in most forms of glaucoma. They work in two key ways:
Reduce Fluid Production
Some drops limit the amount of aqueous fluid the eye produces, lowering pressure from within.
Improve Fluid Drainage
Others help the eye's natural drainage system work more efficiently, allowing excess fluid to flow out more easily.
In many cases, a combination of drops is used to achieve better pressure control. Your consultant will determine the most appropriate approach based on your glaucoma type, severity, and response to treatment.
Types of Glaucoma Eye Drops
There are several classes of glaucoma eye drops, each working differently. Your treatment plan will depend on your glaucoma type, severity, and how your eyes respond.
Prostaglandin analogues
Usually the first choice. Improve fluid drainage and are typically used once daily. Examples include latanoprost, bimatoprost, and travoprost.
Beta-blockers
Reduce aqueous fluid production and are commonly used once or twice daily. Examples include timolol and betaxolol.
Carbonic anhydrase inhibitors
Decrease fluid production inside the eye. Available as drops (dorzolamide, brinzolamide) or oral tablets (acetazolamide) in more acute situations.
Alpha agonists
Work by both reducing fluid production and improving drainage. Examples include brimonidine and apraclonidine.
Combination drops
Contain two medications in one bottle for more effective pressure control and improved convenience for patients using multiple drops.

What to Expect
Frequency of use
Drops are instilled once or multiple times daily depending on the type prescribed. Your consultant will provide clear instructions for each drop.
Regular monitoring
Frequent follow-up appointments are required to monitor intraocular pressure, check the optic nerve, and assess whether the drops are working effectively.
Long-term treatment
Glaucoma cannot be reversed. Treatment is often lifelong, as drops control pressure but do not cure the underlying condition.
Adjustments over time
Your drops regime may be adjusted based on pressure readings, side effects, or progression. Compliance with your prescribed regime is essential for the best outcome.

Is Drops Management Enough?
In early and moderate stages, glaucoma drops are often sufficient to control intraocular pressure effectively. However, if pressure is not adequately controlled despite optimised drops, or if the condition continues to progress, additional treatments may be recommended.
These may include laser procedures such as Selective Laser Trabeculoplasty (SLT), or surgical options such as deep sclerectomy or trabeculectomy. A personalised treatment plan ensures you receive the most appropriate approach for your individual condition.
