When Floaters Become More Than a Nuisance

Vitreous floaters are opacities within the vitreous gel — the clear, jelly-like substance that fills the interior of the eye. As the vitreous ages and gradually liquefies, it can pull away from the retina in a process known as posterior vitreous detachment, leaving behind dense strands, clumps, and membranes that cast shadows on the retina.

For many people, floaters are a minor inconvenience. How Does Vitrectomy Work?

Pre-operative Assessment

The procedure is performed under local (peribulbar) or general anaesthesia, selected according to your clinical needs and personal preference to ensure comfort throughout.

Vitreous Removal

Three micro-incisions are made at the sclera. A high-speed vitrectomy cutter removes the vitreous gel with precision, eliminating the floaters at their source whilst maintaining stable intraocular pressure.

Before

Pre-operative Assessment

Your journey begins with a thorough clinical assessment including biometry, OCT, and retinal examination. Systemic health is reviewed and any medications that may affect surgery are discussed. You will receive full pre-operative instructions covering fasting, transport arrangements, and eye drop preparation.

During

The Procedure

Under local or general anaesthesia, three small entry ports are created at the sclera. The vitreous gel is removed using a high-speed cutter whilst the eye is irrigated to maintain stable pressure. The procedure takes approximately 45 to 60 minutes. You will be comfortable and closely monitored throughout.

After

Early Recovery

Mild redness, blurring, and discomfort are normal in the first few days and settle quickly. Prescribed eye drops protect against infection and manage inflammation. Arrange transport home on the day of surgery. Most patients return to light activities within one to two weeks.

Follow-up

Structured Review

Review appointments are scheduled at one day, one week, and one month post-operatively. Your surgeon monitors intraocular pressure, retinal health, and visual recovery at each stage. Full visual stabilisation typically occurs within two to four weeks of surgery.

Is Vitrectomy Right for You?

  • Patients in good general and ocular health who are able to undergo intraocular surgery.

Pars plana vitrectomy is highly effective. Published studies consistently report floater elimination in over 90% of appropriately selected patients, with significant improvement in visual quality and patient-reported quality of life following surgery.

As with all intraocular surgery, vitrectomy carries risks including cataract formation, retinal tear or detachment, raised intraocular pressure, infection, and, in rare cases, significant vision loss. These complications are uncommon in experienced surgical hands. Your consultant will discuss all risks in full during your consultation.

Cataract formation is the most common long-term consideration following vitrectomy in patients over 50, occurring in the majority within two to three years of surgery. This can be treated straightforwardly with routine cataract surgery if it becomes visually significant.

Vitrectomy is performed under anaesthesia and is not painful during the procedure. Post-operatively, mild aching or discomfort for a few days is normal and is well managed with standard analgesic medication.
